delaeMD Terms of Service

By engaging with our platform, you recognize and accept the respective operational bounds of our distinct tiers of service:

• The Healthspan Blueprint: A standalone, discrete, one-time asynchronous evaluation snapshot. The service terminates upon delivery of your single written risk profile evaluation and physician-reviewed plan. It does not include continuous clinician messaging, continuous care adjustments, or prescription refills.
• The delaeMD Membership: An ongoing care tier providing continuous, longitudinal healthspan tracking and preventive care coordination. This tier provides recurring laboratory test ordering sequences, ongoing data analysis, and messaging touchpoints with your medical team. Membership tiers are subject to recurring monthly or annual fee frameworks as explicitly agreed to at digital checkout.

Clinical services are restricted entirely to individuals who are legal residents of, or physically located within, the State of California at the time of service execution. By submitting data or processing payments, you affirm that you are at least 18 years of age, located in California, and bound to provide complete, transparent, and accurate clinical histories.

You recognize that delaeMD operates under an un-marked-up, radical transparency billing model:

• The Blueprint Blueprint Split: A $59 medical review fee paid to delaeMD, Inc., and a separate $107 baseline screening cost routed entirely and directly to our testing partner, Ulta Labs, with exactly 0% clinic markup or kickback collection.
• Membership Billing Framework: Membership subscriptions incur automated, recurring payments based on your designated plan cycle. You may cancel your membership subscription tier at any time according to the operational cancellation parameters located within your patient onboarding dashboard.

These Terms, as well as any legal conflicts arising from the platform or clinical assessments, are governed exclusively under the regulatory jurisdiction of the State of California. delaeMD, Inc. shall not be held liable for third-party laboratory diagnostics processing latencies, automated connection drops, or errors resulting from a patient's omission of critical health records during onboarding.
